Position: Nurse Manager (RN)
Key Responsibilities:
- Provide oversight, supervision, and support to field staff including CNAs, CHHAs, and other caregivers.
- Conduct patient assessments and develop individualized care plans in accordance with professional nursing standards.
- Collaborate with physicians and multidisciplinary team members to optimize patient care and outcomes.
- Educate patients and families on health maintenance, disease prevention, and care expectations.
- Ensure accurate documentation, compliance with regulatory requirements, and adherence to Joint Commission standards.
- Monitor clinical performance and provide coaching, training, and mentorship to staff.
- Act as a key liaison between patients, families, staff, and the BrightStar Care leadership team.
- Support case management, scheduling, and follow-up to ensure continuity of care.
- Active RN license in the state of Massachusetts .
- Minimum 2 years of nursing experience (home health or acute care experience strongly preferred).
- Prior leadership, supervisory, or case management experience is a plus.
- Strong organizational, interpersonal, and communication skills.
