- Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
- Utilizing the state tracking system HHH (EMR like tool used by DPH) after receiving comprehensive training.
- Completing assessments for clients within 24 hours of their family's arrival into the system.
- Providing on-site medical support as needed by CSR ((Clinical and Safety Risk Assessment) families.
- Assisting CSR families with medical referrals, appointment setup, and access to required medical services.
- Educate patients and their families on health maintenance and disease prevention.
- Ensure accurate documentation and compliance with all regulatory guidelines.
- Active RN license in the state of practice.
- Previous experience on patient admission and doing assessment.
- Exceptional interpersonal and communication skills.
