This role collaboratively develops, implements, and monitors clinical nutrition care for patients receiving enteral and parenteral nutrition as part of the Nutrition Support Team. The position involves assessing medical history, anthropometrics, laboratory data, and insurance requirements to guide appropriate nutrition interventions, ensure continuity of care, and support safe transition to home nutrition therapy. The role also includes patient and caregiver education to promote understanding, adherence, and optimal outcomes with prescribed nutrition support regimens.
Schedule: Mon-Fri 8:30am - 5:30pm / Remote

- Makes recommendations for nutrition intervention when necessary
- Reviews patient medical history, anthropometrics and pertinent lab values to assess home nutrition support protocols and insurance payer requirements
- Counsel patients and caregivers on home enteral and parenteral nutrition therapy. Assesses patients' understanding of and compliance with prescribed diet principles
- Ensure continuity of care with patient's physician, nurses and nutrition support teams to provide high quality patient centered care
- Participate in Performance Improvement Program and ACHC accreditation
- Identifies and promotes nutrition program services to clinical and professional referral base and performs necessary in-service education
- Supervisory Responsibility: No
- Bachelor of Science in Nutrition or related field
- Registered Dietitian with license to practice in assigned state(s); Certified Nutrition Support Clinician preferred
- Minimum of two (2) years of clinical experience required.
- Must have strong communication skills and organizational skills
- High level of self-motivation, initiative, flexibility and ability to work under pressure
**To perform this role will require constant sitting, standing, walking, bending, reaching and typing on a keyboard with fingers. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
