Brighton Park Neighborhood Council (BPNC) is an intergenerational community-based organization dedicated to equity and racial justice. Our mission is to improve the quality of life for the working-class and immigrant populations of Chicago's southwest side through grassroots organizing and providing free and accessible social services and quality programming for youth and adults.

POSITION SUMMARY:

Under the supervision of the Family Services Director, the IWC Case Manager provides case management and advocacy services to support immigrants in successfully integrating into the community. The Case Manager connects clients to immigration legal services, healthcare, education, language programs, citizenship resources, and other community support through BPNC and partner agencies. Services are provided in person, virtually, and at partner locations to promote client stability, self sufficiency, and overall well being.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Responsibilities

• Provide comprehensive and culturally sensitive case management services to all clients, including intake assessment, benefit assessment, financial goal setting, educational goal setting, money management, and financial literacy training.
• Weekly case plan development, progress monitoring, advocacy, and referrals.
• Assist 250 families in a year.
• Meet with clients on a weekly or bi-weekly basis depending on need.
• Connect clients to community resources and follow up to ensure services are accessed and referral outcomes are documented.
• Maintain accurate and complete client records by entering all client interactions and case information into the agency database in a timely manner and in accordance with program requirements.
• Provide all required documentation on time.
• Participate in regular staff meetings, staff training, supervisory sessions, and other duties as assigned by the supervisor.
• Adhere to agency policy, procedures, and the professional code of ethics.

Required Qualifications

• Bilingual Spanish/English
• Drivers License and and reliable transportation
• Bachelor’s degree in Social Work or related field (preferred)
• Knowledge of community resources and counseling/social work practices with high-risk populations.
• Good documentation, skills, and verbal and written communication skills. Ability to establish rapport.
• Ability to motivate others towards achieving goals.
• Ability to work independently with a strong sense of focus, task-oriented, non-judgmental, clear sense of boundaries.
• Ability to work in various settings with culturally diverse families and communities and the ability to be culturally sensitive and appropriate.
• Ability to pass a federal background check before starting the position
• Ability to complete first aid-CPR Certification and Mandated Reporter Certification.
• Must have proficient computer skills

Pay: $50,000.00 - $54,000.00 per year
