Role Description
The Registered Nurse at Brighton Hospice provides direct, holistic nursing care to patients and supports their families throughout the hospice journey. This full-time, traveling role in Farmington, CT includes assessing patient needs, developing and implementing individualized care plans, administering medications and treatments, and monitoring symptoms to promote comfort and quality of life. The nurse collaborates closely with physicians, social workers, chaplains, aides, and other interdisciplinary team members to coordinate care and ensure alignment with the patient’s goals and preferences. Daily responsibilities also include documentation in accordance with regulatory standards, educating patients and families about disease progression and hospice services, and participating in care conferences and on-call rotations as needed.
Qualifications
- Candidates should possess strong clinical nursing skills in assessment, care planning, symptom management, and medication administration.
- Candidates should possess excellent communication, active listening, and interpersonal skills to support patients, families, and team members.
- Candidates should possess skills in collaboration, care coordination, and interdisciplinary teamwork within a hospice or similar care setting.
- Candidates should possess organizational, time management, and documentation skills to ensure accurate records and compliance with policies and regulations.
- Active Registered Nurse (RN) license in the state of Connecticut, in good standing.
- Previous experience in hospice, palliative care, home health, or related fields is highly beneficial.
- Ability to provide compassionate, patient-centered care and to support individuals and families during end-of-life transitions.
- Basic computer proficiency for electronic health records and communication tools; valid driver’s license and reliable transportation may be required for patient visits.
