Job Overview

The Family Consultant serves as the agency’s case manager and the primary liaison for Align Utah. This role is central to ensuring that every client’s treatment and permanency goals are identified, supported, and successfully achieved. Family Consultants are responsible for overseeing all aspects of client progress, coordinating services, and maintaining clear, consistent communication with foster families, caseworkers, therapists, educators, and other professionals.

Family Consultants maintain a caseload of 10–15 clients and are required to provide a

minimum of 4 hours of face-to-face mentoring and support per client each month. This

position requires strong advocacy, proactive case management, and the ability to lead

the care team in supporting each client’s success. Family Consultants must also be able to

transport clients safely as needed and drive to client homes to provide direct support.

General Responsibilities

  • Serve as the agency’s primary liaison for all assigned cases, ensuring clear and

consistent communication between Align Utah, foster families, caseworkers, therapists,

schools, and other providers.

  • Actively oversee and monitor client progress toward treatment and permanency goals,

ensuring timely interventions and support as needed.

  • Provide mentoring, guidance, and emotional support to each client through regular

face-to-face interactions (minimum of 4 hours per client per month).

  • Adhere to Align Utah policies, state licensing requirements, and confidentiality

standards.

  • Complete all required documentation (progress notes, incident reports, training forms,

monthly progress reports, Child and Family Team Meeting notes, transition & aftercare

plans, discharge summaries, etc.) in a timely and accurate manner.

  • Attend and actively participate in all required meetings, including school meetings, Child

and Family Team meetings, therapy sessions, court hearings, and agency training.

  • Facilitate scheduled home visits with families to review and implement S.M.A.R.T. goals.
  • Transport clients safely to appointments, visits, or activities as needed.
  • Support conflict resolution and implement de-escalation strategies as needed.
  • Respond to grievances in alignment with Align Utah’s Grievance Policy.

Responsibilities to Clients

  • Advocate for clients’ individual needs in all settings, including foster homes, schools, and

Child and Family Team Meetings.

  • Ensure treatment plans, behavioral goals, and permanency objectives are consistently

monitored, updated, and supported.

  • Provide direct mentoring and skill-building to help clients achieve behavioral,

educational, and treatment goals.

  • Guide youth in the Transition to Adult Living (TAL) process by connecting them with

resources for housing, employment, budgeting, and independent living skills.

  • Monitor client safety and well-being, ensuring foster families report any accidents,

emergencies, or significant behavioral issues.

  • Implement approved intervention and de-escalation strategies when clients display

challenging behaviors.

Qualifications and Education Requirements

  • Must be 21 years of age or older (per state requirements).
  • Strong communication skills (verbal, written, and interpersonal).
  • Ability to mentor youth, problem-solve effectively, and work with diverse families.
  • Ability to pass all background checks required by the State of Utah.
  • Must be able to manage a caseload of 10–15 clients while ensuring treatment and

permanency goals are consistently met.

  • Valid driver’s license, reliable vehicle, and ability to transport clients safely as needed.

Preferred Skills

  • Experience mentoring or coaching youth in care.
  • Knowledge of trauma-informed care and positive behavior interventions.
  • Ability to work independently and as part of a team.
  • Strong organizational skills to balance paperwork, client visits, and team coordination.

Job Type: Full-time

Pay: $21.00 per hour

Expected hours: 40 per week
