What are the responsibilities and job description for the Police Social Work Case Manager position at Bright Harbor Healthcare?
On P.O.I.N.T. embeds social workers in the police departments to collaborate effectively with officers and address the community’s needs. On P.O.I.N.T. provides an easy access point for officers to refer those in need without the burden of knowing all available programs, services, and the modality to access them. Part of the referral process is indicating any concerns for safety or the need for a joint outreach with the social worker and officer.
Arrive Together works to partner with Law Enforcement in identifying community residents with an unmet mental health, substance use or social service need, and reducing police contacts via social work intervention.
Position Title: Case Manager
Position Type: Full Time; 35 Hours Weekly (some evenings required)

Location: Ocean County Southern Region (Stafford, Little egg, and Long Beach Island)
Department: On P.O.I.N.T.
Responsibilities:
- Report to assigned Ocean County Police Departments throughout the week
- Accept referrals from participating municipal law enforcement agencies and triage needs
- Provide needs assessment, linkage and brief case management to referrals
- Work as part of team
- Conduct community outreaches independently and as part of team
- Maintain information/data spreadsheets
- Coordinate and consult with law enforcement officers and law enforcement command staff
- Complete documentation as required and communicate with other program staff
- Participate in law enforcement meetings and education
- Work with student interns to enhance their educational experience
- Bachelor’s Degree in Social Work or related field
- Experience working with Substance Abuse
- Proficient with computer programs including Excel, Power Point, Word
- Experience and understanding of Law Enforcement
