At Bright Day ADHC we provide therapy, recreational programs, and social services to the frail, elderly, and adults over 18 years of age. We specialize in caring for those with physical, cognitive or mental impairments. Bright Day ADHC is licensed by the State of California, offering comprehensive medical and nursing care to the local community.
Bright Day ADHC/ CBAS is seeking to hire a (preferred Spanish speaking) Social Worker who is responsible for conducting initial assessments and six-month reassessments of each participant's mental, social and emotional status, develop individualized plans of care, provide group and/ or individual counseling to participants, assist in providing case management services and other duties as needed. Must have a Bachelor's degree from an accredited school in Social Work, with two years of experience providing social work in one or more fields of aging, health or long term care services.
