Position Overview

Duties
- Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to ensure comfort and dignity.
- Support residents with medication administration, vital signs monitoring, and maintaining accurate care plans in compliance with HIPAA and other regulations.
- Provide patient observation to identify changes in health or behavior, ensuring timely communication with healthcare professionals.
- Facilitate engaging activities that promote social interaction, cognitive stimulation, and overall well-being tailored to individual needs.
- Prepare nutritious meals aligned with dietary restrictions and preferences while maintaining cleanliness in the kitchen area.
- Operate equipment such as Hoyer lifts and assist with heavy lifting tasks safely to prevent injury.
- Maintain a clean and safe environment by performing regular cleaning duties in residents’ living spaces and common areas.
- Document all care activities meticulously, including patient monitoring, medication administration, and incident reports.
- Support residents with specialized needs such as Alzheimer's disease, dementia care, autism spectrum disorders, or other developmental disabilities through personalized approaches.
- Collaborate with interdisciplinary teams including social workers and healthcare providers to develop and implement effective care plans.
- Uphold confidentiality standards by adhering to HIPAA guidelines during all interactions and recordkeeping.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly preferred but not required.
- Demonstrated knowledge of patient care techniques such as CPR, first aid, patient monitoring, and vital signs measurement.
- Experience working with individuals who have developmental disabilities, Alzheimer’s disease, dementia, autism spectrum disorder or require memory care is advantageous.
- Familiarity with long-term care practices including hospice care and home health services enhances your ability to provide comprehensive support.
- Ability to perform physical tasks involving heavy lifting and the safe operation of assistive devices like Hoyer lifts is necessary.
- Strong communication skills combined with patience and empathy are essential for building trusting relationships with residents.

Requirements

- Must be dependable and calm under pressure
- Comfortable working independently in a home setting
Preferred Experience
- DSP, CNA, home health aide, or similar caregiving experience
- Experience with feeding tubes, oxygen, or breathing equipment (not required but a plus)
- Experience with nonverbal or high-support-needs clients
