Essential Functions
- Conduct assessments of patients' physical, mental, and functional abilities.
- Contribute to individualized treatment plans based on assessments.
- Implement and monitor treatment plans, adjusting as needed.
- Provide education to patients and their families regarding safe disposition planning to least restrictive environment.
- Collaborate with other healthcare professionals, such as physicians, nurses, and physical therapists, to provide comprehensive care.
- Maintain accurate, timely and up-to-date patient records and progress notes.
- Attend staff meetings, continuing education programs, and professional development activities to stay current with new techniques and best practices.
