We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant with a passion for dementia-focused care. In this hybrid role, you will provide care from a home or clinic-based setting with a mix of telehealth, while also helping grow our footprint in senior living communities over time. What You’ll Do

As a Provider with Brigade Health, you will:

• Conduct and support annual cognitive assessments and occasional home visits in partnership with affiliated medical groups
• Provide dementia-focused care through both in-person visits and telehealth sessions
• Manage chronic conditions and behavioral symptoms associated with cognitive decline
• Manage post discharge visits, either in-person or via telehealth, and support appropriate cost of care outcomes for patients as the leader of a care team
• Engage in family education and advanced care planning conversations
• Collaborate with our clinical team and help develop care pathways as we expand into senior living settings
• Deliver proactive and compassionate care that builds long-term patient and caregiver trust
  
  

You Are

• A licensed Nurse Practitioner (NP) or Physician Assistant (PA)
• Experienced or interested in geriatrics, neurology, behavioral health, or dementia care
• Comfortable in clinic, home, and virtual care settings, with strong communication and clinical assessment skills
• Eager to be part of a mission-driven team and contribute to program development
