With our residents in mind, we are seeking an experienced Social Worker for our Bridgewater location. Schedule is as follows: Monday-Friday 8:30AM-4:30PM
Duties/Responsibilities:
- Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each resident
- Assess psycho/social well-being of patients.
- Coordinate and participate in Care Plan Meetings
- Acting as a liaison with social, health and community agencies
- Discharge Planning
- Develop relationships with physicians and families
Qualifications/ Skills:
- Must be able to read, write and communicate in English
- Experience in long term care preferred
- Experience with screening, assessment, and planning for common social services needs
- Strong interpersonal communication skills with exceptional active listening abilities
- Highly empathetic, non-judgmental, and open-minded
- Experience in a collaborative team environment
Education and Experience:
- Certified or Licensed Social Worker in the State of NJ
- Bachelors Degree (Required) in Social Work or Psychology. Master’s Degree preferred
- 1 years' experience in clinical social work
- A problem-solving orientation and a flexible and positive attitude

- Proficient PC skills
