Lead a Mental Health Team in HUDSON COUNTY, NJ - Team Leader

Under the direction of the Director, the Team Leader is responsible to oversee the service recipients’ goal attainment process, over the course of the person’s involvement in services, in order to assist each individual to move through the program and to become fully integrated into the community. The Team Leader has primary responsibility for supervision of direct care staff and the day-to-day operation of the program.

Position Overview:

Bridgeway is seeking a Team Leader who wants to lead a dynamic, multi-disciplinary team consisting of mental health clinicians, psychiatrist, registered nurses, peer specialists and co-occurring (behavioral and substance use) counselors.  The ideal candidate should love working in an environment that offers flexibility and a collegial atmosphere.  As part of a multi-disciplinary, core services team, the  team leader will assess treatment needs, complete biopsychosocial assessments, provide  and other direct individual counseling clinical to help ameliorate symptoms for adults with psychiatric disabilities. This position is 70-80% community based. The candidate who fills this position will also have administrative oversight of the team.

The Team Leader – will assume the following responsibilities:


o As team clinical and administrative supervisor in the frequent monitoring and assessment of the mental health 

status of people served as well as related variables, including significant others, the home and the community in 

which the person served lives. 

o In the development of comprehensive assessment, coordination of all other assessments, and an individualized 

service plan. Ensures documentation is done on time and with quality to ensure adherence to all 

DMHAS/Medicaid standards. 

Provides: 

o Clinical supervision, leadership and direction to the PACT team, emphasizing a peer clinical supervision 

approach and using individual supervision as needed. Provides monthly supervision for direct reports and 

seeks out monthly supervision from own supervisor. 

o Monthly individual, side-by-side sessions accompanying an individual staff member to meet with persons 

served, family/significant others, and/or other service providers in regularly scheduled or crisis meetings 

o Monthly structured meetings with staff as a group to address issues that cannot be addressed during the 

daily, clinical, or treatment planning meetings or during the side-by-side sessions, to develop performance 

and training goals, and teach of specific interventions/skills 

o Promotion of cross-training and education among various disciplines on the team 

o Education and direction to the team to ensure compliance with applicable rules and regulations 

o Assessment of service recipients stated concerns, mental illness symptoms, and behavior in response to 

medication and monitors for medication side-effects during the provision of observed self-administration and 

during ongoing face-to-face contacts 

o Medication education to both individuals and family/significant others with service recipient consent. 

o Coordination of services with other community MH and non-MH providers, and other medical professionals 

o Ensuring that cases requiring more in-depth analysis and discussion among PACT team members are 

reviewed at clinical case review meetings, to be held at least once per month 

o Individual counseling to a designated number of individuals receiving PACT services. Shall provide 

psychotherapy for the resolution of symptoms of psychiatric disability, psychosocial stress, relationship 

problems, or difficulties in coping with the social environment 

o Consult and refer to other services beyond scope of PACT team’s expertise (with written waiver) 

o Rapid and flexible response to crisis, including but not limited to: accompanying person to local screening center 

or psychiatric emergency service and remaining with the person during the assessment process 

o 24 on-call coverage at all times, with the ability to respond immediately to individuals by phone or in person 

o Appraisal and assessment of, but not limited to, individuals’ daily living skills, and teach individuals needed skills 

in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, 

self-advocacy and maintenance of housing 

o Support to assist individuals to find and maintain employment and/or educational training 

o Oversight of utilization of agency equipment and resources 

o Support to the service recipient’s family and other members of his/her social network to help them manage 

 Education and License: 

o Master’s Degree in Counseling, Psychology, Social Work or related area required;  

o 2 years’ post-Bachelor’s experience working with individuals with serious and persistent mental illness 

Driver’s License, Vehicle: 

o Valid driver’s license required 

o No more than one moving violation within the past 12 months 

o Vehicle required
