POSITION DESCRIPTION:
This position is responsible for the daily operations of the Rockland County Jail Re-entry Program. Provide Case Management for persons in the Rockland County Jail and post-release that is person-centered in alignment with the necessities of their return home from Correctional Facilities. Assist persons to increase stabilization, pro-social skills and supports and to connect participants to necessary community resources.
QUALIFICATIONS:
General
- A Bachelor’s degree (B.A.) from a four-year college or university; or 3 to 4 years related experience and/or training; or equivalent combination of education and experience
- Bilingual preferred
- A valid Driver’s License
- Must be able to travel locally and regionally for trainings
Job Specific
- CASAC preferred
- Interactive Journaling Facilitator
(may be completed after hire)
- Understanding of NIC’s Transition from Prison to Community Model
- Experience assisting formerly incarcerated persons
- Ability to communicate with Behavioral Health providers, County and Correctional Agencies specifically the Rockland County Sheriff’s Office, Probation, Rockland County Court system, Community based providers
- Must be person-centered and passionate; able to build relationships, provide positive reinforcement and empowerment
- Maintain a creative, flexible, non-judgmental approach
- Strong ability to set boundaries and clear expectations
- Demonstrate ability to work in a fast-paced environment
- Efficient with time management and strong organizational skills
- Demonstrate ability to work independently and as a member of a team
- Good written and oral communication skills
- Proficiency in Microsoft Suite
- Required to disclose associations and/or family members currently incarcerated or under any type of Community Supervision in NYS
- Satisfactory background check required

- Provides case-managed services to individuals for successful community Re-entry of those released from Rockland County Jail to meet stabilization and criminogenic needs
- Provides case-managed services to persons in Rockland County Jail Re-entry program
- Processes referrals and manages all case file documentation
- Field visits with participants for engagement and case conferencing
- Responsible for procedures from pre-release through release and beyond
- Completes Re-entry Intake/Screening/Assessments
- Chairs Conflict-Free Case Conferencing to develop person-centered service and treatment plans
- Facilitates Interactive Journaling and maintains group attendance records and documentation
- Attends Re-entry Task Force meetings
- Responsible for duties and tasks associated with Group meetings
- Maintain communication with Rockland County Department of Mental Health
- Attends other workgroups, collaborative meetings and case conferences at supervisors request
- Additional duties and responsibilities per Supervisor as needed
