Role Summary:
Promotes and restores residents quality of life by providing care as determined by an assessing nurse and the Individual Service Plans (ISPs).

Reports To:
Nurse and Director of Operations

Job Function:
Promote resident’s independence by teaching resident, caregivers, friends, and family to understand condition, medications, and self-care skills as directed by a nurse; and working on strategies with other disciplines.

Must Have Skills:
Graduation from an approved program and current certification by the State of New York as a Certified Home Health Aide.

Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.

Document patient care services appropriately by charting thoroughly and completely on all required forms including medication and treatment administration records. Record all entries in client documentation book in an informative and factual manner. Use only authorized established abbreviations, when recording information.

Interact with other disciplines to identify and follow through with all areas of concern identified and communicating with caregiving staff on other shifts to ensure continuity of care.

Ensure that patient care needs are met daily by working collaboratively with other caregiving staff.

Ensure nursing supplies inventory by assisting patient; maintain accurate and complete records; use equipment and supplies as needed to accomplish job results.

Foster teamwork by accepting responsibility for own actions, working cooperatively with other staff and voluntarily assisting co-workers and understanding and utilizing the unique contributions of team members.

Communicate openly and effectively by treating all people with dignity and respect, expressing differences of opinion in a constructive manner, listening carefully before responding to a concern or recommendation

Learn continuously by taking initiative to improve job skills and seeking opportunities to learn new skills. Remain current with in-service requirement

Physical Demands:
Stands, walks most of the shift. Must be able to stoop, push, pull, lift, and transfer patients weighing 50 to 250 pounds using mechanical lifts. Able to wear gloves and other protective equipment. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure the requirements of this position can be fully met. Must be in good general health and demonstrate emotional stability. Must be able to lift a minimum of 50 pounds.

Working Conditions:
Generally pleasant, but possible exposure to unpleasant sights, sounds and odors. May have to deal with unpleasant or uncomfortable situations involving difficult patients and/or families. Must be able to cope with the mental and emotional stress of the position.

Operation of Equipment:

  • Use only the equipment you have been trained to use.
  • Operate all equipment in a safe manner
  • Use only the equipment and supplies necessary to do the job. Do not be wasteful.
  • Report defective equipment to your supervisor.
  • Inform the supervisor of your equipment needs.

Job Type: Full-time

Pay: $17.00 - $25.00 per hour
