Bridges Community Action Partnership is seeking a passionate Resident Service Coordinator for our Madison and Shelby County offices to empower residents, connect families to vital resources and help build stronger, healthier communities throughout the county.

ESSENTIAL FUNCTIONS:

  • Maintain and manage a caseload of up to 50 program participants.
  • Assess resident needs and develop service plans to support self-sufficiency and stability.
  • Coordinate and monitor delivery of supportive services through community partners, tribal departments, and other organizations.
  • Connect participants to appropriate resources and track service outcomes.
  • Communicate effectively with residents, partners, and stakeholders to support program goals.
  • Plan and facilitate educational workshops and community events, including the facilitation of Getting Ahead classes.
  • Maintain accurate case records, documentation, and required reports. Demonstrate proficiency in software used to track KPls and program outcomes.
  • Knowledge of procedures and eligibility requirements for federal and applicable state entitlement programs.
  • Understanding of federal nondiscrimination laws and legal liability related to service coordination.
  • Knowledge of aging-related issues and elder services.
  • Familiarity with disability services and mental health resources.
  • Awareness of substance abuse issues, including drug and alcohol use among older adults.
  • Working knowledge of local supportive services for youth, adults, seniors, and individuals with disabilities.
  • Ability to advocate for low-income families, older adults and individuals with disabilities.
  • Strong skills in organization, problem-solving and achieving results.
  • Ability to deliver eligible activities in compliance with nondiscrimination requirements.
  • Experience working with underserved communities.
  • Flexibility for other duties as assigned.

QUALIFICATIONS AND REQUIREMENTS:

  • College degree or significant relevant work experience. A Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling is preferable, although any other completed college degree is fully acceptable.
  • High-school diploma or equivalency
  • At least two years of experience in social services, counseling, or related field
  • Ability to be an advocate for all clients we serve.
  • Demonstrated knowledge of emergency services programs and community resources.
  • Valid driver’s license and be insurable under agency policy.

PHYSICAL REQUIREMENTS:

  • Requires standing, bending, reaching, sitting and working with office equipment such as computer, telephone, etc.
  • Lifting up to 50 lbs. occasionally
  • Operating an agency vehicle occasionally

ENVIRONMENTAL REQUIREMENTS:

  • Typical office work environment and sound level
  • Extended hours may be required during critical workloads
