We are seeking a dedicated and compassionate Home Health Aide (HHA) to provide exceptional in-home care to individuals requiring assistance with daily activities and health management. Duties
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, ensuring comfort and dignity.
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to support ongoing health assessments.
- Support medication administration following prescribed protocols while maintaining accurate medical documentation in electronic health record (EHR) systems.
- Provide specialized care for clients with dementia or Alzheimer's disease, including memory care strategies and behavioral management.
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively.
- Assist with mobility using tools like Hoyer lifts or transfer belts to prevent falls and promote independence.
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with HIPAA regulations.
- Collaborate with healthcare professionals to coordinate home & community care services tailored to individual needs.
- Maintain thorough documentation of patient care activities in compliance with medical records standards and HIPAA privacy rules.
- Support hospice care clients by providing compassionate emotional support alongside physical assistance.
Skills
- Strong patient observation skills to identify changes in health status or behavior promptly.
- Extensive experience working in assisted living facilities, nursing homes, or home health environments.
- Proficiency with EMR (Electronic Medical Records) and EHR systems for accurate documentation.
- Knowledge of medical terminology relevant to home health care practices.
- Ability to assist with complex procedures such as catheter care and medication administration safely.
- Certifications in CPR (Cardiopulmonary Resuscitation) and first aid are required or preferred.
- Experience working with individuals with developmental disabilities or cognitive impairments like dementia or Alzheimer’s disease.
- Familiarity with hospice care principles, dementia care techniques, and memory support strategies.
- Competence in performing vital signs measurement accurately and reporting findings appropriately.
