FULL TIME HOSPICE CASE MANAGER
We are looking for a Hospice Case Manager with strong knowledge in hospice and palliative care who can work directly with our team of nurses, doctors and social workers to coordinate services. The successful candidate will be responsible for managing the medical case management process to provide comprehensive services to patients and families that are at the end of life or have a serious illness that is managed at home.
Responsibilities:
- Work with the case managers in hospice to coordinate long-term care needs, provide support to patients and families, and evaluate care and treatment.
- Analyze case reports from hospital team.
- Review care plans of patients who are being discharged from the hospital.
- Provide support services to patients and families.
- Communicate with medical specialists about patient's care plan.
- Work with hospitals & doctors about patients' medical care plan.
Job Type: Full-time
Pay: $91,000.00 - $95,000.00 per year
