Job Overview
Brenda’s Place LLC is seeking an experienced and motivated 245D Program Manager to help lead and oversee our Home and Community-Based Services (HCBS) program. This leadership role is responsible for ensuring full compliance with Minnesota DHS 245D regulations while driving program quality, staff development, and person-centered service delivery.
You will serve as the Designated Manager and provide overall operational leadership, ensuring services are effective, compliant, and impactful.
Responsibilities
- Serve as Designated Manager in accordance with MN DHS 245D requirements
- Oversee all aspects of program operations, compliance, and service delivery
- Ensure ongoing compliance with Minnesota Statutes Chapter 245D and 245A
- Provide leadership, supervision, and support to staff and clients
- Develop and implement staff training and professional development
- Monitor and ensure accurate documentation, service plans (CSSP), and progress notes
- Oversee incident reporting, investigations, and corrective action plans
- Conduct internal audits and ensure readiness for DHS licensing reviews
- Collaborate with case managers, families, and community partners
- Promote a trauma-informed, person-centered approach to services
- Travel Locally to client homes and service locations as needed
This role includes both administrative leadership and hands-on program support, which is standard for 245D leadership roles overseeing HCBS programs.
Requirements
- Must meet qualifications for Designated Manager under MN 245D
- Bachelor’s degree in Human Services or related field (preferred)
- Minimum 3 years of supervisory experience in home community based services or similar programs such as Assisted Living
- Strong knowledge of MN DHS 245D policies, compliance, and audits
- Experience with staff training, coaching, and leadership
- Excellent organizational, communication, and leadership skills
- Valid driver’s license and reliable transportation
- Employment is contingent upon successfully passing a Minnesota DHS background study in accordance with 245D requirements
