What you will do

- Provide skilled interventions for dressing, bathing, grooming, toileting, transfers, energy conservation, caregiver training, home safety, adaptive equipment, and functional independence.
- Help patients and caregivers build practical routines that support safe discharge, reduced fall risk, and continued independence at home.
- Communicate patient progress, barriers, and changes in condition promptly with nursing, PT, Speech Therapy, case management, physicians, and family caregivers.
- Complete timely, compliant documentation using reliable software and available AI assistance.
- Participate in care coordination and discharge planning.
