Breaking Barriers Case Management is a growing provider dedicated to supporting individuals with daily living needs and promoting independence. We are building a strong, reliable team and looking for compassionate and dependable staff.
Responsibilities:
- Assist clients with daily living activities (bathing, dressing, grooming)
- Provide meal preparation and light housekeeping
- Support clients in the community and with appointments
- Monitor and report client condition and behavior
- Complete required documentation (service logs and notes) daily
Requirements:
- Must be 18 years or older
- High school diploma or GED (preferred)
- Valid driver's license and reliable transportation
- Ability to pass background check
- Dependable and professional
- Strong communication skills
Preferred Qualifications:
- Experience in caregiving, home care, or direct support
- CPR/First Aid certification (or willing to obtain)
