POSITION SUMMARY

Performs selective medical services such as examinations, formulation of care plans, under the direction of the physicians. Provides professional nursing care to patients following established standards, practices and healthcare techniques.  Demonstrates competence in making professional judgments pertaining to patient care.  Assists providers in patient care and education. Actively participates as a member of the healthcare team to engage patients, families, and caregivers in the coordination of patient care.

ESSENTIAL FUNCTIONS

  1. Performs Medicare Annual Wellness Visits for patients who qualify for the services, per Medicare guidelines, under supervision of a provider.
  2. Maintains acceptable patient volumes as per corporation’s benchmarks.
  3. Follows guidelines, as defined by the US Preventative Services Task Force, and make recommendations of wellness screening tests and vaccinations based on those guiding principles. 
  4. Performs different screening exams such as PHQ2/9, Snellen vision screen, and whisper test along with screening for fall risk, urinary incontinence, tobacco, drugs, and alcohol. 
  5. Alerts physicians and/or APCs when concerns are identified during the annual wellness visit to ensure that patients receive the most appropriate level of care that matches their need at the time.
  6. Travels to multiple primary care locations, depending on patient volumes, to perform Annual Wellness exams. 
  7. Provides patient with written plan of care immediately following each visit.
  8. Provides general nursing care to patients.  Administer prescribed medications, vaccines, and treatments in accordance with nursing standards, US Preventative Services Task Force recommendations, and Standing Orders. 
  9. Screens patients for appropriate information and observes, records and reports patient’s condition and reaction to drugs and treatments to the provider.  Instructs patients in collection of samples and tests.
  10. Provides appropriate education and instructions to the patient and family regarding medication and treatment protocol.
  11. Advise patients regarding health and illness prevention and recommend community resources to meet patient and family needs if applicable.
  12. Contacts patients to provide education on the importance of the annual wellness visit and schedules the appointment for the patient.
  13. Coordinates with staff who may be assisting in the scheduling process for annual wellness visits, which includes developing and adjusting scripting that is used.
  14. Maintains and monitors data base of patients who qualify for the annual wellness visit, including attempted contacts, refusals, and scheduled appointments.
  15. Performs and documents various tests and procedures as directed by the provider. Documents patient information, test and examination results and care provided in the patient’s paper or electronic health record.
  16. Evaluates and responds appropriately to urgent or emergency situations. 
  17. Provides timely, accurate, and complete documentation of care provided in the patient’s medical record to ensure timely billing of services. 
  18. Participates in quality improvement activities and processes.
  19. Contacts patients to relate test results and care instructions, according to provider instructions.
  20. Reviews laboratory results and other patient information.
  21. Coordinates patient referrals, appointments, testing and admissions to other facilities. 
  22. Obtains proper insurance company referral or authorization for required visits or testing.

 

NON-ESSENTIAL FUNCTIONS

Performs related and miscellaneous duties as assigned.

 

MINIMUM REQUIREMENTS

Education:

  1. Graduate of an accredited school of nursing.

Experience: 

  1. One year nursing experience in primary care preferred.

Knowledge, Skills, Abilities:

  1. Knowledge of nursing principles and skills needed to provide quality patient care and treatment including examination, diagnostic and treatment procedures, medications effects on patients and patient education principles.  
  2. Knowledge of common safety hazards and precautions.  
  3. Knowledge of office procedures and medical terminology.  
  4. Advanced knowledge of ICD-10 and CPT coding guidelines.  
  5. Ability to communicate clearly and effectively and read, understand and follow written and verbal instructions of the physician.  
  6. Must be able to react calmly and effectively during emergency situations.  
  7. Must have the ability to identify and analyze problems, prioritize, make confident quick decisions and recommend effective solutions.  
  8. Ability to administer a variety of medications and treatments and utilize various types of equipment for examination and treatment procedures.


 License/Certification/Registration:

  1. Texas nursing license required: Registered Nurse
  2. Valid Texas Driver’s license.

Meets the BLS/CPR standards renews every two years

12. Have constant need (66% - 100% of time) for typing or writing. 

13. Have rare need (less than 1% of time) for driving or operating equipment. 

Lifting/Carrying: 

1. Have rare need (less than 1% of time) to lift/carry over 150 pounds. 

2. Have rare need (less than 1% of time) to lift/carry 76 - 150 pounds. 

3. Have rare need (less than 1% of time) to lift/carry 51 - 75 pounds. 

4. Have occasional need (1% - 33 % of time) to lift/carry 26 - 50 pounds. 

5. Have occasional need (1% - 33 % of time) to lift/carry 10 - 25 pounds. 

6. Have frequent need (33% - 66% of time) to lift/carry under 10 pounds. 

Vision Requirements: 

1. Have constant need (66% - 100% of time) to be able to see detail. 

2. Have frequent need (33% - 66% of time) to be able to see up to two feet away.                                           

Hearing Requirements: 

1. Have constant need (66% - 100% of time) to be able to hear other personnel in order to communicate effectively. 

Environment- Working Conditions: 

1. Have constant need (66% - 100% of time) to work indoors. 

2. Have rare need (less than 1% of time) to work outdoors. 

3. Have rare need (less than 1% of time) to work in confined space. 

4. Have occasional need (1% - 33 % of time) for exposure to fumes, chemicals, solvents, etc. 

5. Have rare need (less than 1% of time) for exposure to extreme temperatures.
