Responsibilities
- Conduct comprehensive mental health assessments, including evaluation of cognitive, emotional, and behavioral status, alongside monitoring vital signs to detect clinical changes.
- Develop and implement individualized, evidence-based care plans in collaboration interdisciplinary team members.
- Educate patients and family members on diagnosis, treatment plans, medication adherence, symptom management, and coping strategies to promote recovery and self-management.
- Provide psychosocial support and guidance for managing psychiatric symptoms, pain, and co-occurring medical conditions.
- Maintain thorough, timely, and compliant documentation of patient progress and interventions using electronic health record (EHR) system.
Skills
- Strong understanding of pain management techniques.
- Ability to take vital signs accurately and assess patient conditions effectively.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare team members.
- Strong organizational skills with attention to detail in documentation and patient care.
- Proficient in post-operative and post-acute inpatient care.
- Experience in administering injections safely and effectively.
- Knowledge of tube feeding procedures.
- Ability to assess and monitor vital signs accurately.
- Familiarity with ostomy care practices.
