Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs.
- Administer medications and injections accurately following established protocols while ensuring compliance with HIPAA and medical documentation standards.
- Manage patient care plans by collaborating with physicians, case managers, and other healthcare professionals to optimize health outcomes.
- Document all patient interactions thoroughly using EMR (Electronic Medical Records) or EHR (Electronic Health Records) .
- Provide education on health coaching topics such as nutrition, medication administration, dementia care (including Alzheimer’s and memory care), and discharge planning for patients and their families.
Experience
- Proven experience working as an LPN in diverse healthcare settings such assisted living and LTC.
- Experience working with individuals with disabilities or developmental delays in settings like assisted living or home & community care is advantageous.
- Knowledge of physiology, anatomy, pharmacology principles related to medication administration and patient assessment techniques.
- Ability to operate medical imaging devices or perform specimen collection/processing when required.
- Strong understanding of HIPAA regulations along with proficiency in phlebotomy, tube feeding management, and basic life support (BLS). Pay: $25.00 - $27.50 per hour
