You’ll work closely with a supportive, dedicated team with accessible and experienced supervision, and have the full backing of the entire organization to ensure your success.
This position is primarily home-based, with one required office day per month (in North Brunswick) after onboarding and training. This role will have cases in Monmouth, Ocean, and Middlesex counties and would like a candidate living in that area.
Key Responsibilities:
- Provide support coordination/case management for individuals eligible for DDD services.
- Develop and monitor personalized service plans for adults with developmental disabilities.
- Assist individuals in exploring and accessing community supports.
- Build and maintain positive relationships with families and community providers.
- Travel required within New Jersey; a valid NJ driver’s license and reliable transportation are necessary.
Qualifications:
- Bachelor’s degree (any field; social services degree preferred).
- At least one year of experience (including volunteer work) with adults with developmental disabilities.
- Strong organizational skills and ability to work independently.
- Ability to meet deadlines and collaborate effectively with a team.
- Previous experience in social services preferred but not required.
- Valid driver’s license and dependable transportation required.
- Must pass background checks through DDD and the Brain Injury Alliance of New Jersey.
