Job Summary:
Currently licensed as a Licensed Practical Nurse in New York State
- Two years of related LPN work experience and/or training
- One year of related work experience in a Skilled Nursing Facility/Assisted Living or Dementia Unit
- Effective oral and written communication skills
- Ability to function independently
- Computer literate
- Trained in basic first aid
- Able to learn and follow pre-established Wellness Department procedures as a team member with the main focus on resident care and medication administration
- Maturity, emotional and mental stability
- Good verbal and written communication skills and personal initiative
Responsibilities
- Ensuring the well-being and safety of the Assisted Living residents.
- Conduct resident evaluations and provide nursing services as needed.
- Coordinate with healthcare team to develop and implement individualized care plans.
- Ensure compliance with HIPAA regulations and maintain patient confidentiality.
- Collaborate with residents and families to facilitate a smooth transition to Assisted Living
- Utilize computer skills to document patient information accurately.
