What are the responsibilities and job description for the MDS Coordinator with RN License position at Bradley Healthcare & Rehabilitation Center?
Looking for an experienced full-time RN, MDS Coordinator to join our team. MDS Coordinator experience is required. This position will oversee the completion and submissions of timely MDS Assessments and Care plans. Must be able to report changes in patients' condition that impact documentation ensuring assessments of patients. Interviewing patients and families, working with the interdisciplinary team to assure timely completion of resident assessment protocols and patient care plans. Complete calendar for MDS time frame and ensure timely completion by all departments
Must possess an RN license and have 2 years of nursing experience, preferably in long term care facility. Must have MDS coordinator experience and knowledge in this field. Drug Free. Equal Opportunity Employer
