Licensed Practical Nurse

Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to residents in a small 39 apartment assisted living community. This role requires a strong commitment to resident care, effective communication skills, and the ability to work collaboratively with both healthcare and non-healthcare professionals to ensure the highest standards of care.

Duties

• Administer medications and treatments as prescribed by physicians.
• Perform patient assessments and monitor vital signs.
• Assist with daily living activities for patients, ensuring comfort and dignity
• Collaborate with registered nurses and other healthcare team members to develop and implement patient care plans.
• Provide education and support to patients and their families regarding health management.
• Conduct triage assessments in emergency situations.
• Maintain compliance with HIPAA regulations and infection control protocols.
• Participate in the development of discharge planning for patients transitioning from acute care settings.
• Engage in ongoing professional development to enhance nursing skills and knowledge.

Qualifications

• Current LPN license in good standing.
• Experience in long-term care, or assisted living preferred
• Experience working with diverse populations, including those with Alzheimer's
• disease.

Every other weekend 7am-7pm

Monday, Thursday and Friday evenings either 2pm-7pm or 4pm-7pm

• Ability to perform basic life support (BLS) and maintain current certification.
• Excellent communication skills and a compassionate approach to patient care.
