Multi-Specialty group located in Bristol County is recruiting for an experienced Nurse Practitioner to join their team providing care to homebound patients. We have adopted a proactive approach to delivering care at home (through house calls) and facilitating the transitions from hospital-to-home. This is a 32-hour workweek, seeing on average 6-8 patients per day.
The Nurse practitioners responsibilities will include:
Visiting them at home
- They will diagnose conditions; order testing; prescribe and manage medications; educate patients and care takers.
- They will interact with PCPS, visiting nurses and Case Managers
To qualify for this position the Nurse Practitioner must:
- Have 2 years working in a similar setting
- Licensed in the State of Ma, Rhode Island license is a
- Strong communication skills necessary to interact with other providers
Job Types: Part-time, Permanent
Pay: $91,500.00 - $108,000.00 per year
