Position Description
Title: Case Manager
Reports To: Program Director
FLSA Status: Non-Exempt $40,500/yr
Primary Function
The Case Manager will provide individualized support and guidance to participants in the Kindergarten to Career Coaching Program. This role focuses on case management, resource coordination, and progress monitoring to ensure participants achieve academic success, career readiness, and personal growth.
Key Roles (Essential Job Responsibilities)
Case Management:
- Conduct intake assessments to identify participants’ needs and develop individualized support plans.
- Monitor progress in academic performance, career readiness, and executive function development.
- Serve as a liaison between families, schools, program staff, and community partners to coordinate resources and services.
Resource Coordination:
- Connect youth and families to resources such as tutoring, internships, counseling, and technical skills training.
- Develop partnerships with community organizations to expand available resources.
Documentation and Reporting:
- Maintain case notes, records, and program data for evaluation and grant reporting purposes.
- Assist in tracking key program outcomes and preparing updates for stakeholders.
Other:
· Perform other relevant duties as requested.
Skills/Knowledge Required:
- Bachelor’s degree in social work, education, or related field. (preferred)
- Experience in case management or youth services.
- Strong organizational skills and attention to detail.
- Ability to engage effectively with youth, families, and community partners.
