As a YouthConnect Social Worker, you will play a crucial role in our holistic approach to comprehensive youth services. Working in collaboration with a team of social workers, you will have the opportunity to engage with families in a confidential, voluntary, and free capacity. Our unique model, built on a clinical framework, allows us to meet youth where they are, providing support at a pace that suits their individual needs.
Upon police referral, you will be responsible for conducting assessments and reaching out to youth and families. Remarkably, 85% of the families we contact choose to pursue our services, which are organized into three tiers: Therapy Services, Clinical Case Management, and Resource Coordination. This tiered approach empowers families to define and pursue their individual goals, ensuring a customized and effective intervention.
