Role Description
This is a full-time, on-site role for a Home Health Physical Therapist at Bowes In Home Care. Based in Crystal Lake, IL, the Physical Therapist will assess, plan, and execute individualized treatment programs for patients receiving in-home care. Primary responsibilities include evaluating patient conditions, implementing rehabilitation plans, focusing on improving mobility and functionality, and providing guidance to patients and their families. The role involves collaboration with a multidisciplinary team to ensure patient-centered care and successful recovery outcomes. This job will require travel to patients homes in Crystal Lake and neighboring cities.
Qualifications
- Proficiency in Physical Therapy practices, Rehabilitation techniques, and Home Care services
- Strong knowledge of Medicine and Patient Care standards in a home health setting
- Exceptional interpersonal and communication skills to interact effectively with patients, families, and healthcare teams
- Valid Physical Therapy license in the state of Illinois
- Ability to manage time effectively while handling multiple tasks in a varied work environment
- CPR certification
- Experience with electronic medical records (EMR) systems is beneficial
