Home Health Physical Therapist (PT)/Physical Therapist Assistant (PTA) – Flexible Schedule

In this role, you will serve as both clinician and coordinator — delivering hands-on therapy in patients’ homes while overseeing the full scope of their rehabilitation plan. You’ll work closely with patients, families, and our interdisciplinary healthcare team to ensure each individual achieves their highest level of mobility, independence, and quality of life. Responsibilities
- Provide skilled physical therapy services to patients in their homes in accordance with the established plan of care.
- Assess patients’ physical abilities, mobility, strength, balance, and endurance to develop individualized treatment plans.
- Implement therapeutic exercises, gait training, manual therapy, and interventions to improve function and safety.
- Educate patients, families, and caregivers on safe mobility techniques, fall prevention, equipment use, and home safety modifications.
- Monitor, evaluate, and document patient progress, updating treatment plans as necessary to achieve optimal outcomes.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
- Ensure accurate, timely, and compliant clinical documentation per agency and regulatory requirements.
- Provide training in the use of assistive devices and adaptive equipment to maximize independence.
- Participate in case conferences, team meetings, and ongoing care coordination.
- Maintain professional licensure and stay current with best practices in physical therapy and home health care.
Requirements
- Current Illinois license to practice as a Physical Therapist.
- Graduate of an accredited Physical Therapy program.
- At least 1 year of home health or comparable clinical experience (preferred).
- Strong assessment, critical thinking, and communication skills.
- Current CPR certification.
- Valid driver’s license and reliable transportation.
- Requires travel within reason to initiate evaluations or SOCs
- Proficiency in electronic documentation (Kinnser experience a plus, carefficent).
