What are the responsibilities and job description for the Family Preservation Technician position at Bowen Health?
Duties/Responsibilities:
Below are the primary duties and essential functions of an employee in this position. While not all tasks may be performed by every employee in this role, all employees are expected to perform related tasks as needed. - Document patient progress notes.
- Review and update patient information and chart documentation.
- Complete protective factors assessment.
- Assist families in maintaining safety in the home. Advocate, teach, demonstrate, monitor, and model focus area skills. Focus areas may include cleanliness of home, maintaining appropriate supervision, building parenting skills, maintaining a budget and household utilities, and other needs.
- Regularly communicate with Bowen DCS Manager and DCS Family Case Manager (FCM) regarding progress and safety concerns.
- Assist families in building/increasing natural support systems and protective factors.
- Assist families in learning and developing new skills to improve functioning, safety, and independence.
- Ensure patients are linked with appropriate community
- Ensure patients are linked with needed healthcare providers.
- Ensure patients are addressing the goals that are important to them and/or their referral source.
- Ensure families are making progress, and progress is appropriate documented so cases can be closed/discharged in an appropriate amount of time.
- Participate in DCS child and family team meetings.
- Transport clients as needed and appropriate to their care.
- Complete all clinical documentation by time frames identified in policy.
- Other duties as assigned.
Physical Requirements:
- Occupational Exposure Risk Classification: The normal work routine involves minimal exposure to blood, body fluids or tissues.
- The normal work routine has a minimal ergonomic risk for repetitive motion impairment.
- The position may require a minimal amount of physical exertion (bending, lifting, twisting) in the work area.
Required Qualifications:
- Education: High School Diploma or GED.
- Experience: Prefer two (2) years of experience in caseload services to children and families.
- Certificate or Professional License:
- CPI (Crisis Prevention Intervention) - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
- BLS (Basic Life Support) and First Aid - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
- Valid driver’s license and active auto insurance coverage.
- Satisfactory results from criminal and driving record background checks
- Must be a minimum of 21 years of age.
