Job description:
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will possess a strong background in physical therapy and demonstrate expertise in various treatment modalities, including myofascial release and outpatient care. This role involves working with patients in an outpatient setting to develop and implement effective care plans that promote recovery and enhance quality of life.
Responsibilities
- Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
- Develop individualized treatment plans based on patient assessments and goals.
- Implement therapeutic interventions, including exercise programs, manual therapy techniques, and modalities such as Trigger Point Therapy.
- Monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes.
- Collaborate with other healthcare professionals to coordinate care and discharge planning for patients.
- Educate patients and their families about treatment options, home exercises, and lifestyle modifications to support recovery.
- Maintain accurate documentation of patient evaluations, treatments, and progress in compliance with regulatory standards.
- Stay current with advancements in physical therapy practices and participate in ongoing professional development.
Requirements
- Valid state licensure as a Physical Therapist.
- Strong knowledge of physical therapy principles, practices, and techniques related to orthopedics, geriatrics, and acute care settings.
- Experience with discharge planning and developing comprehensive care plans for diverse patient populations.
- CPR/BLS Certification
- Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams.
- Ability to work independently as well as collaboratively within a team environment.
- Proficiency in using electronic health record (EHR) systems for documentation purposes is a plus.
