Job Summary:
The Community Health Worker (CHW) serves as a trusted liaison between patients, healthcare providers, and community resources to improve access to care and support better health outcomes. This position focuses on assisting patients with financial needs—such as insurance enrollment, sliding fee applications, and payment plans—while also providing education, outreach, and connection to social services.
Key Responsibilities:
Assist patients and families with financial assistance programs, including Medicaid, Medicare, Marketplace plans, and sliding fee applications.
Collaborate with front office, billing, and finance teams to create manageable payment plans.
Provide culturally appropriate health education and connect patients to community resources such as food, housing, transportation, and employment services.
Serve as a liaison between patients and the healthcare team to enhance communication and care coordination.
Identify and address barriers to care, including unmet social needs.
Participate in health promotion activities, community outreach, and quality improvement efforts.
Advocate for patients to promote health equity and fair access to care.
Other duties as assigned
Qualifications:
High school diploma or equivalent required; some college and healthcare experience preferred.
Certified Community Health Worker (CHW) or eligible for Arkansas certification through the Experiential Track, or the ability to obtain certification within one year of hire.
Knowledge of insurance programs, financial assistance processes, and community resources preferred.
Experience working with diverse populations and maintaining cultural sensitivity.
Proficiency in electronic health records and Microsoft Office or Google Workspace tools.
Bilingual (English/Spanish or other languages) preferred but not required.
Active BLS certification required.
