What are the responsibilities and job description for the Family or Internal Medicine Physician - LaGrange, Georgia position at Borinquen Medical Center?
- Provide evidence-based primary care to 10-15 patients daily, focusing on prevention, chronic and acute care, and disease management.
- Build strong patient relationships and develop personalized biopsychosocial care plans with a multidisciplinary team, including health coaches.
- Conduct comprehensive health assessments, physical exams, diagnostic testing, and prescribe medications, utilizing onsite lab and pharmacy services.
- Drive patient engagement through health evaluations, coaching, and health promotion activities, aiming to improve overall health outcomes.
- Participate in organizational and quality improvement initiatives, meetings, and team collaborations, including after-hours coverage rotation.
- Ensure all patient and contractual needs are met, maintaining high standards of care aligned with the organization's mission.
- Achieve clinical and population health benchmarks by enhancing patient health status and satisfaction.
- Qualifications include MD or DO, board certification, 3 years primary care experience, and current unrestricted medical license. Experience with EMR systems and team-based care is required.
