- Conduct initial OT evaluations and develop individualized plans of care under physician orders
- Provide skilled occupational therapy focused on ADL retraining, adaptive equipment, home safety, and functional independence
- Reassess patient progress and modify plans of care every 30 days
- Complete OASIS documentation accurately and on time in compliance with Medicare and ACHC standards
- Coordinate with the clinical team and patient's physician on care planning; supervise COTAs as needed per MA practice standards
- Active Massachusetts OT license in good standing
- Graduate of an ACOTE-accredited occupational therapy program
- 1 year of home health or acute care experience preferred; motivated candidates with strong clinical backgrounds considered
- Familiarity with Medicare OASIS documentation (training available)
- Reliable transportation, valid MA driver's license, and current auto insurance
- Available for at least 2–4 visits per week (flexible on timing)
