Overview
We are seeking a compassionate and motivated Assessment Social Worker to support older adults through psychosocial assessments, short-term case assistance, and program engagement. This role plays a key part in enhancing the well-being of older adults by identifying appropriate services and connecting them with enriching, supportive programs.
Responsibilities
-Conduct in-home psychosocial assessments for new and ongoing older adult participants
-Discuss program opportunities and determine the best fit based on the individual's interests, needs, and abilities
-Provide short-term case assistance, including information, referral, and advocacy
-Maintain accurate and detailed client records using a CRM/database system
-Collaborate with colleagues to initiate participation in relevant programs and services
-Facilitate or co-facilitate one or more psychoeducational or support groups each year
-Manage a caseload of 15–20 Friendly Visiting matches, conducting quarterly check-ins and offering support
-Support community-wide intergenerational volunteer programs and on-site events
-Cover the Information & Referral line as needed
-Participate in special projects and additional duties as assigned
-Travel to clients' homes using public transportation (travel expenses covered)
Requirements
-Licensed as an LMSW in the state of New York
-Passionate about supporting the older adult population
-Skilled in clinical assessment, interpersonal communication, group facilitation, and advocacy
-Organized and comfortable working both independently and collaboratively
-Strong written and verbal communication skills
-Familiar with local aging and community resources (a plus)
-Experienced with volunteers and/or community-based programming (a plus)
-Available to work up to 8 Sundays or evenings per year for events and programs
-Tech-savvy with Microsoft Office Suite; Salesforce or other CRM/database experience a plus
-Proficiency with Microsoft Word, Excel, Teams, Zoom, and Outlook
-Experience with client tracking in databases; Salesforce knowledge is a plus, but training is available
