What You’ll Do
- Perform patient assessments and develop care plans tailored to individual needs.
- Coordinate care and communicate with physicians, families, and your care team.
- Monitor patient progress and adjust plans to support the best outcomes.
- Provide education and support to patients and families for confident home care.
- Enjoy a balance of field visits and collaboration with your team.
- RN license in Ohio and CPR certification.
- 1–2 years of nursing experience (home health preferred).
- Strong communication and organizational skills.
- Passion for helping patients live well at home.
