Job Overview
We are an established physician-led family medicine practice in Bolton, CT seeking an experienced Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team.
This is an excellent opportunity for an APRN who wants the benefit of a 4 day work week, predictable hours, a supportive physician, and a more personal practice environment rather than a large corporate medical system. Strong applicants who wish to work less than a full 4 day week will be considered.
In this vital role, you will deliver comprehensive primary and specialized care to diverse patient populations across all ages, from pediatrics to geriatrics. Your expertise will support patient health through thorough assessments, diagnosis, treatment planning, and ongoing management. You will collaborate closely with physicians, specialists, and other healthcare professionals to ensure exceptional patient outcomes in a collaborative, warm, innovative environment. Position
- 4 days per week/34 hours
- Monday-Friday schedule with one weekday off
- No weekends
- No hospital call
- 100% outpatient family medicine
- Work closely with an experienced family physician
- Independent patient care with physician support when needed

Duties
- Conduct detailed patient assessments including physical examinations, vital signs, and medical history reviews using EMR with AI assist.
- Diagnose acute and chronic conditions across various specialties including internal medicine, geriatrics, pediatrics, behavioral health, and infectious disease care.
- Prescribe medications and treatments in compliance with DEA regulations and collaborative practice agreements.
- Provide telehealth consultations utilizing telehealth platforms to expand access to care for remote or underserved populations.
- Document all patient interactions thoroughly while maintaining HIPAA compliance and accurate medical records management.
Compenstion & Benefits

Qualifications
- Current CT licensure as a Family Nurse Practitioner and national certification.
- Demonstrated experience working with diverse populations including children and adults.
- At least 2 years of clinical experience preferred.
- Strong clinical skills encompassing physical examinations.
- Strong communication and interpersonal skills
- Caring, personable and patient focused
- Interested in developing long-term relationships with patients
- New graduates with strong clinical training may be considered.
