- Key responsibilities include: conducting weekly in-home nursing visits, performing physical assessments, ensuring home safety, managing medications, and overseeing caregivers.
- Collaborate with team members to improve services, plan and evaluate care, and serve as a clinical resource in clients' homes.
- Participate in staff hiring, training, documentation review, and quality improvement initiatives.
- Support community outreach and ensure compliance with agency policies and scope of practice.
- Graduate of an approved nursing program, BSN preferred.
- Current RN license, with a minimum of two years nursing experience, including one in home care.
- CPR certification; supervisory or case management experience is a plus.
