Key Responsibilities
- Perform thorough assessments of patients’ physical, social, and psychological needs, integrating input from team members, patients, and families.
- Develop individualized care plans through critical thinking and patient involvement, including education and follow-up support.
- Direct and coordinate care delivery by delegating tasks, managing resources, and prioritizing patient needs in collaboration with the care team.
- Evaluate patient responses to interventions, adjust care plans accordingly, and ensure patient and family satisfaction.
- Maintain professionalism by adhering to legal and ethical standards, documenting accurately, and complying with regulatory requirements.
- Valid RN license in the practice state; Basic Life Support (BLS) certification.
- Strong skills in patient care delivery, interdisciplinary collaboration, documentation, communication, critical thinking, and care coordination.
- Minimum requirement of a current RN license; BSN preferred, with mandatory attainment within four years if less than 12 months of experience.
- Ability to see, hear, communicate effectively, manipulate equipment, lift, and assist with patient mobility.
- Frequent walking, standing, and maneuvering in busy environments; may involve driving and handling heavy items.
