Job overview
We are seeking a Concierge Care Coordinator to serve as a central point of connection between our patients, clinicians, and care partners. This is a high-touch, detail-driven role for someone who takes pride in creating a seamless, calm, and professional experience for patients navigating complex healthcare needs.

What you will do
As a Concierge Care Coordinator, you will support patients throughout their care journey while helping ensure smooth clinical operations. Key responsibilities include:
- Serve as a primary point of contact for patients via phone and secure messaging, triaging requests with warmth, discretion, and sound judgment
- Coordinate appointments, referrals, follow-ups, and care logistics to ensure continuity and timeliness
- Communicate with outside providers, facilities, and specialists, including medical records exchange and referral coordination
- Manage and maintain accurate clinical documentation within our EMR (Elation)
- Track lab and imaging orders, follow up on results, and support clinicians in review workflows
- Support daily clinic operations, including office organization and inventory management
- Participate in team meetings, contributing to workflow improvements and patient experience initiatives
- Collaborate closely with physicians, medical assistants, and leadership to uphold our high standards of care
About you
Education & experience
- Associate’s or Bachelor’s degree required
- Prior experience in primary care, concierge medicine, or integrative/functional medicine is strongly preferred

Compensation & benefits
- Base salary range: $65,000–$80,000, commensurate with experience
