BlueWave Medicine is located in the Presidio, a historic 1,500-acre national park site overlooking the Golden Gate Bridge. We are a close-knit, collaborative practice committed to excellence in care and innovation in delivery. We’re looking for a full-time Care Navigator to help us provide a seamless and supportive experience for our patients.
As a Care Navigator, you will play a key role in guiding patients through their care journey. This position supports clinical operations and patient communication in a high-touch, tech-enabled practice. Your core duties will include:
- Warmly answering and triaging phone calls and emails from patients.
- Coordinating appointments and follow-ups to ensure smooth scheduling.
- Managing communication with outside providers and facilities, including document sharing and referral coordination.
- Organizing and uploading electronic documents into our EMR system.
- Facilitating medical records requests and ensuring timely delivery to and from referring providers.
- Following up on lab and imaging orders and ensuring results are received and reviewed.
- Maintaining accurate and up-to-date records using Elation EMR, including chart preparation and health maintenance tracking.
- Supporting clinicians by preparing electronic charts before visits.
- Monitoring and restocking office inventory as needed to support team workflows.
- Actively participating in staff huddles and team meetings to improve patient care and workflow.
- Collaborating with clinicians, medical assistants, and leadership to support optimal care and patient satisfaction.
- Associate’s or Bachelor’s degree required.
- Exceptional attention to detail and follow-through.
- Strong communication skills with a warm, professional demeanor.
- Adaptable, self-directed, and comfortable in a fast-paced environment.
- Previous experience in primary care, concierge medicine, or integrative care settings is a plus.
- Solid understanding of HIPAA and patient privacy best practices.
