Role Description
This is a full-time on-site role for a Case Management Nurse located in Lincoln, NE. The Case Management Nurse will be responsible for coordinating patient care, managing discharge planning, and ensuring the appropriate utilization of resources. The role involves working closely with patients to plan and implement individualized care plans and support their healthcare needs effectively.
Qualifications
- Experience in Case Management and Discharge Planning
- Knowledge of Utilization Management
- Strong Nursing skills and foundational knowledge in Medicine
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Experience in a healthcare setting, preferably in a Federally Qualified Health Center (FQHC)
- Active Registered Nurse (RN) license
- Bachelor's degree in Nursing or a related field is preferred
