Now Hiring: Foster Care Caseworker (SWAN) - Bachelor Degree in Human Services (including but not limited to: Psychology, Sociology, Criminology or other Social Services Degree)
Join our Team! Blueprints is hiring a Foster Care Caseworker. The Caseworker will Represent Blueprints within the social services community and serves as a liaison in collaborative agency efforts. Maintain a caseload of clients, establish positive relationships with each client and foster family and ensure all program and treatment goals are being addressed and attained. Develop and implement an individual service plan for each assigned client through collaboration with foster care staff and placing agency representative.
Brief Description of Duties:
- Implement recruitment plan and screen potential foster parents
- Design and maintain outreach materials/advertisements with Communications Coordinator
- Provide foster parenting information via calls, walk-ins, home visits, and materials (brochures, applications)
- Submit inquiries and applications to Homefinders to maintain active files
- Handle and follow through with foster care referrals
- Facilitate recruitment activities (displays, booths, events, speaking engagements, advertising)
- Build and maintain relationships with referral sources and community resources
- Manage caseload; build positive relationships with participants and foster families
- Participate in 24-hour on-call rotation for emergencies/crisis intervention
- Develop and implement individualized service plans (ISPs) with staff and agencies; complete documentation and billing
- Monitor participant progress; hold conferences; recommend care continuation or revisions to ISPs
- Complete and file documentation (intake, case notes, visitation, progress reports, assessments); update referring agencies
- Attend court hearings and agency meetings related to participants
- Maintain bi-weekly face-to-face contact with each participant
- Deliver socially necessary services (family visits, transportation, parenting support)
- Ensure compliance with federal, state, local laws, credentialing standards, and agency policies
- Perform other duties as assigned by supervisor/director
- Work some evenings and weekends
