Schedule: 8:00am-4:30pm with occasional on-call
Position Summary
The PACE Registered Nurse performs all activities related to registered nurse services to achieve program goals and/or contracted service requirements at the PACE Center and the participant’s home. This includes collecting comprehensive patient data, identifying problems and needs, prioritizing care according to risk and acuity, collaborating with IDT to develop individualized care plans, collaborating with other disciplines, and making appropriate referrals. Direct care may include wound care, medication administration, and other appropriate nursing interventions.
The PACE Registered Nurse will monitor patient responses to interventions and adjust if appropriate and provide education to patients, families, and caregivers.

Position Qualifications
- Is a graduate of an accredited school of registered nursing.
- Is currently licensed as Registered Nurse through the State Board of Nursing.
- Must have at least one year of experience working with the frail and elderly.
- Food handling permit, to be completed upon hire.
- Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
- Complies with accepted professional standards and principles.
- Possesses and maintains current CPR certification.
- Have satisfactory references from a nursing school, previous (or current) employers, and/or professional peers.
- Have excellent observation skills, good nursing judgment, and communication skills.
- Ability to provide good written documentation in a timely manner
- Is self-directed with the ability to work with little supervision.
- Is flexible and cooperative in fulfilling role obligation and has the ability to work within an interdisciplinary team.
