What are the responsibilities and job description for the Intensive Case Manager/Wraparound Facilitator position at Bluebonnet Trails Community Services?
As a valued member of the Bluebonnet Community Services Youth and Family services team, this professional position is develops a comprehensive assessment of the youth and their families, working in full partnership with team members to develop a plan of care, overseeing implementation of the plan, identifying providers of services or family based resources, facilitating child and family team meetings, and monitoring all services authorized for a child's care.
The Wraparound Facilitator assures that care is delivered in a manner consistent with strength-based, family driven, youth guided, and culturally competent values, offers consultation and education to all providers regarding the values and principles of the model, monitors progress toward treatment goals, and ensures that necessary data for evaluation is gathered and recorded.
The person in this position requires an aptitude for working with people who have a mental health diagnosis as well as evidence-based practices in the promotion of recovery for individuals with severe and persistent mental illnesses. This position allows opportunities for skilled professionals to use their knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis as well as wellness and recovery-oriented systems of care.
The person in this position will have a passion and aptitude for working with youth who have severe emotional needs. Knowledge of medications, case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the behavioral health field is critical for success in the position.
This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, and perform other duties as assigned with a positive attitude.

Minimum Qualifications
A Bachelors degree from and accredited college or university with a major or equivalent course work for a major in a social, behavioral, or human service field and knowledge of mental illness.
Must have a valid Texas Drivers License and maintain approved drivers status throughout employment.
One year of experience in mental health assessment and/or crisis intervention and screening.
Masters degree in psychology, social work or a related human service field.
Bilingual preferred.

Essential Functions
- Builds and maintains a therapeutic relationship with youth and families through engagement.
- Provides wraparound services along with linking individuals with community support.
- Performs assessments with individuals identifying their strengths and needs.
- Completes Wraparound Plans developing person-centered goals and objectives with individuals.
- Completes documentation in a timely manner.
- Has initiative and works well in ambiguity
- Engages individuals based on personalized plan
- Completes Assessments within expected time frame (90 days)
- Completes Wraparound Plans to guide treatment
- Is a member of an interdisciplinary treatment team
- Has access to ongoing Training
Hourly Salary Range: $22.22 - $31.47
Differential Detail
- Up to $4.00/hour, tenure with BTCS
- $1.00/hr Masters degree
- $0.75/hr Bilingual
- Up to $2.00/hour, previous community center experience in a similar position
- $1.00/hour, advanced licensure
- .50/hour, LCDC
- $200 Monthly Service Delivery Bonus
