What are the responsibilities and job description for the Case Manager position at Bluebonnet Trails Community Services?
This position delivers high quality professional recovery-oriented case management and skills training services to people with severe and persistent mental illnesses. This position is supported by an interdisciplinary team made up of professional staff including nurses, peers, psychiatrists and others, focused on providing state of the art integrated care.
The person in this position will utilize their expertise to provide training in skills that primarily focus on furthering an
individuals independent living, community skills, and overall wellness and recovery. Services provided by the person in this position should promote community integration, increased stability within the community, as well as an improved quality of life and implementation of self-determination for individuals served, as appropriate. Services provided by this position also include activities and skills training designed to address barriers and strengths related to an individuals recovery and wellness. Service delivery will occur face-to-face or through televideo in the community or in-clinic.

Minimum Qualifications
A Bachelors degree from an accredited college or university with a major or equivalent course work for a major in a social, behavioral, or human service field.
Knowledge of mental illness.
Must have valid Texas drivers license and maintain approved driving status throughout employment.
Preferred Qualifications
One year of experience in mental health assessment and/or crisis intervention and screening.
Masters degree in psychology, social work, or a related human service field.
Bilingual preferred, Spanish and/or ASL.

- $1.00/hour, Associate or Advanced Licensure
- $1.00/hour, Two years previous experience
